1. Quick wins (no visual loss)

Shadow Quality
Dynamic shadows from characters, enemies, and skill effects are constantly recalculated in the Diablo IV engine, and in zones with high enemy density their cost grows noticeably. Lowering to Medium keeps isometric combat readable without the cost of High/Ultra.
Skill/Spell Effects Density
The expansion's expanded skill system lets you stack more simultaneous particle effects than ever, especially in builds built around area damage. This setting directly controls that particle density, and it's precisely the main cause of most FPS drops in crowded fights.
Reflections Quality
Reflections in puddles, wet surfaces, and ice in the new zones carry a noticeable GPU cost that's rarely appreciated from the game's characteristic distant isometric view.

Texture Quality
Character, gear, and environment textures across Sanctuary cost mostly VRAM rather than raw compute power, so High is usually enough on most modern GPUs.
View Distance
Controls how much environmental and enemy detail is rendered in the expansion's open zones; at High/Ultra the cost rises in large outdoor areas without much being noticed from the isometric view.
Global Illumination
Dynamic global illumination recalculates light propagation in dungeons and the expansion's new zones, especially in scenes with many active skill light sources at once.
Post-Processing
Groups bloom, vignetting, and color corrections applied full-screen; at its highest level its cost rises more than it improves the readability of combat.
Anti-Aliasing (TAA)
TAA smooths character and environment edges well, but at its highest level adds a sampling cost that competes with the rest of the graphical effects, especially at higher resolutions.
