How to improve FPS in Pathologic 3 (PC)

Pathologic 3 plunges us back into the agony of the town in the Russian steppe, this time from the perspective of Bakhtin before the events of Pathologic 2. Ice-Pick Lodge has upgraded its engine to Unreal Engine to sustain a pictorial art direction, almost like moving oil paint, backed by a volumetric fog and suspended dust system that wraps every infected street. Beneath that visual layer runs an NPC simulation that never stops: every villager follows their own routine, sick or not, whether you're there to see it or not. That combination of dense atmosphere and living simulation is gorgeous, but also demanding, and not every rig runs it equally well out of the box. Here we go over which settings to tweak so the game can breathe without your FPS suffocating along with it.

⚠️ Known for: Its pictorial volumetric fog and the constant simulation of NPC routines, which keeps the CPU working even on the quietest streets of the village.

1. Quick wins (no visual loss)

Start here. Each one adds a little, but together they give +42% free FPS.

Volumetric Fog/Dust

Recommended: Medium · Visual impact: High · Consensus: The community agrees that Medium keeps 90% of the original atmosphere while losing relatively little detail.
+14% FPS

The fog and suspended dust are Pathologic 3's visual signature, but they're also one of the game's biggest GPU hogs, especially outdoors with a lot of depth of field. Dropping this to Medium preserves the oppressive feel without rendering as many overlapping layers of volumetric particles.

Shadow Quality

Recommended: Medium · Visual impact: Medium · Consensus: Highly recommended setting on mid-range GPUs, considered one of the best value-for-performance options in the game.

Unreal Engine's dynamic shadows are constantly recalculated due to the day/night cycle and NPCs moving through their routines, which makes them costlier than usual. Dropping to Medium softens the outlines without losing the pictorial contrast.

Anti-Aliasing (TAA)

Recommended: Medium · Visual impact: Low · Consensus: Recommended to combine with upscaling instead of maxing out TAA.
+8% FPS

Unreal Engine's TAA helps disguise aliasing on the town's hand-painted textures, but at its highest level it adds a notable internal-resolution cost. Dropping to Medium keeps edges reasonably clean without doubling the frame cost.

Post-Processing/Color Grading

Recommended: Medium · Visual impact: Medium · Consensus: Most players prefer to keep it on Medium to avoid losing visual identity.

Color grading is an essential part of Pathologic 3's desaturated, sickly tone, so it's not worth disabling entirely. Dropping to Medium turns off secondary post-processing layers without touching the base color palette.

Texture Quality/Streaming

Recommended: High · Visual impact: Low · Consensus: Recommended to keep it on High except on GPUs with less than 6GB of VRAM.

Unreal Engine's texture streaming manages available VRAM well, so dropping below High rarely pays off except on cards with very limited memory.

2. Medium impact settings

Here's where most of the FPS is. Minor visual impact, major performance impact.

NPC Simulation Density

Recommended: Medium · Visual impact: Medium · Consensus: Essential setting for anyone with a CPU bottleneck, more decisive than almost any graphics option.
+16% FPS

This is Pathologic 3's most distinctive setting: it controls how many NPCs simultaneously keep their full routine going in the village, somewhat similar in spirit to Kingdom Come Deliverance's simulation. Dropping to Medium reduces that background load without emptying the streets of life.

Volumetric Fog/Dust

Recommended: Low · Visual impact: High · Consensus: Only recommended on very tight hardware; most players stick with Medium if performance allows.
+20% FPS

For very limited systems, dropping the volumetric fog to Low removes much of the light-scattering cost from dust particles, at the expense of losing atmospheric depth outdoors.

Shadow Quality

Recommended: Low · Visual impact: High · Consensus: Considered an emergency setting rather than a general recommendation.
+14% FPS

Dropping shadows to Low drastically simplifies shadow map resolution and reduces the dynamic shadow distance, which is especially noticeable indoors with candles and dim light.

Anti-Aliasing (TAA)

Recommended: Low · Visual impact: Medium · Consensus: Recommended to prioritize upscaling over lowering TAA to Low.
+10% FPS

On Low, TAA reduces its temporal accumulation window, which generates somewhat more visible aliasing on fine edges like the lines of wooden buildings.

Post-Processing/Color Grading

Recommended: Low · Visual impact: High · Consensus: Not widely recommended except as a last resort, due to the high visual cost relative to the benefit.
+5% FPS

On Low, much of the post-processing that gives the game its pictorial, desaturated look is lost, leaving a flatter, more generic image.

3. Upscaling (DLSS / FSR / XeSS)

The biggest gain in the game. Compatible with almost any modern GPU.

DLSS

+35% FPS

On NVIDIA RTX GPUs, DLSS 2/3 is the strongest option for Pathologic 3: it reconstructs the fine detail of the hand-painted textures well and pairs especially nicely with the game's volumetric fog, which tends to hide any reconstruction artifacts.

FSR

+28% FPS

FSR 2 works as a cross-platform alternative for AMD and Intel, with a good balance between sharpness and performance in Quality mode. In scenes with lots of dust in motion it can show a bit more ghosting than DLSS.

4. Tips by GPU

NVIDIA

  • •Enable DLSS in Quality mode before touching graphics settings: on RTX it's the most efficient way to gain FPS without losing sharpness in the volumetric fog.
  • •Update to the latest Game Ready drivers; Unreal Engine 5 receives frequent optimizations that directly affect fog and dynamic shadows.
  • •If you have an RTX card with Frame Generation, save it for calm exploration stretches; in areas with lots of simulated NPCs it can introduce some perceptible latency.

AMD

  • •Use FSR 2 in Quality mode as a first performance measure, especially on RX 6000-series cards or lower.
  • •Keep Smart Access Memory enabled if your CPU+GPU combo supports it; it helps particularly in scenes with more simultaneously simulated NPCs.
  • •Check for recent Adrenalin drivers, since Unreal Engine 5 often gets stability patches specific to RDNA in volumetric shaders.

  • •Install the game on an SSD; texture streaming and NPC routine loading benefit a lot from low access times.
  • •Close background processes before playing: NPC simulation adds constant CPU load, and any extra process competes directly for those cores.
  • •If you notice microstutter when entering new zones, increase the page file size; it helps cushion asset streaming spikes.

5. Known game issues

FPS drops when crossing between town districts

When moving from one zone to another, the engine has to load and activate the routines of a new group of NPCs while also managing texture streaming, causing brief frame drops.

Stuttering from shader compilation in the first session

As in many recent Unreal Engine titles, the first time certain fog or lighting effects load, a brief hitch can appear while shaders compile.

On GPUs with little video memory, combining Texture Quality on High with Volumetric Fog on Medium or higher can cause VRAM saturation and micro-stutters.

Why does my FPS drop so much even though I have a powerful GPU?▾
Pathologic 3 depends heavily on the CPU because of its constant NPC routine simulation, not just graphical power. If your processor is older, lowering NPC Simulation Density usually helps more than any graphics setting.
Is it worth enabling DLSS or FSR from the start?▾
Yes, enabling upscaling in Quality mode is one of the game's best-value changes: it gains a good amount of performance, and the volumetric fog hides any small reconstruction artifacts very well.
Which setting should I lower first on a laptop or entry-level GPU?▾
Start with Volumetric Fog/Dust and Shadow Quality, the two graphics settings that free up the most FPS. If your bottleneck is the CPU, add NPC Simulation Density on Medium.
